As far as the top grade goes: Souths went from minor premiers to wooden spooners in 1989-90. The bulldogs almost reversed this last year. Western Suburbs were 'spooners in 1933, premiers in 1934 Canterbury went from premiers 1942 to spooners in 1943. There might be more, but these are a few examples.
